Trillium Health Partners promotes quality health and independence. With these goals in mind, the following information has been compiled for client and family use. The following resources are options for planning the next steps. Each of these options are beneficial in planning the transition to home and many are financially friendly. More information about these options is available through the navigating resources at McCall Centre for Transitional Care.
1. Local Health Integration Networks (LHIN), previously known as CCAC, will help you arrange next step care for your living by:
- Decide who receives care
- Determine the level of care you need and for how long.
- Determine If you qualify for Ontario government assistance for a wide range of services in your home and community
- Answers questions about health concerns or community resources
- Coordination of in-home services
- Application to day programs
- Applications to Long Term Care (LTC)
2. Milton District Hospital is used as temporary holding area for Alternative Level (ALC) of care.
ALC patients no longer need acute care hospital services but are unable to live independently and are most often waiting for placement in a LTC facility.
3. Bridges to Care provides these medically supervised services to the patient at home and provides the following services:
- Skilled Nursing Care
- Pregnancy Care
- Respite Care
- Senior Care
4. Discharge Home Option
Available for patients in hospitals, but can have their health needs managed in a LTC facility Clients have the option to be discharged home from McCall Centre for Continuing Care in which they will be given priority status for Long Term Care Placement based on their personal level of care. This option is recommendable for clients with various care needs. Resources and referrals to appropriate home care services are available. Such services include:
- Personal Support Worker (PSW) services- dressing, bathing, etc.
- Nursing services
- Occupational Therapy (OT) and Physiotherapy (PT)
- Dental services
- Medic Alert (pamphlet available from navigating resources at McCall Centre)
- Home retirement living (pamphlet available from navigating resources at McCall Centre)
